Reports, including anonymous reports, may be submitted online at any time using the reporting button. Reports go directly to the Associate Dean for Administration.
Emergency situations should be reported to local law enforcement by calling 911.
This form should NOT be used for an urgent situation requiring immediate response. If you are on the campus and experiencing an emergency, call the police at 911.
This form SHOULD be used to report any incidents covered under the Sexual Misconduct, Harassment, Sexual Harassment and Stalking policies and Non-Discrimination statement. This includes harassment, discrimination, and/or relation based on actual or perceived membership in a protected class, as well as incidents of sexual violence (sexual harassment, sexual assault, intimate partner violence, sexual exploitation, or stalking). The report is submitted directly to the Associate Dean for Administration who will reach out within 24-48 hours when the institution is open to offer support and resolution options.
Those IIRP employees designated as responsible persons must identify themselves. Any other person may submit this form anonymously but should understand this may significantly limit the institution's ability to respond.
Please provide as much information as possible for all individuals or organizations involved in the situation, including any witnesses or others with knowledge of the incident.
To allow for the most thorough response, please provide as much detailed information as possible when responding to the questions below.
